Lab Issue Report Form

This form is used by PSU lab attendents to report technology issues with hardware in the general access computer labs. Fields in bold, area info & issue type are required.

Lab Ticket Form

Odin Username: (Must be valid)

Machine number: (e.g. "3", "4,7,9", "All", or "N/A")

Describe the problem in detail:

What troubleshooting has been done?

Does this happen to all users?

Any other relevent information observed by you or the user?
